As of June 30, 2026, Dr. John Paulsel has retired from the practice of medicine.

You may request a copy of your medical records or have them transferred to a non-MCH physician. To do so, please complete and sign the “Authorization to Release Medical Records” form, available on the MCH Medical Records webpage. Upon receipt of your signed form and payment for copying and mailing, we will promptly follow your instructions.
