Signing up for MyChart is simple. Request an activation code from your doctor’s secretarial staff or check-in staff during your visit or go to the MyChart website and select “Sign Up Now.”

MCH leases EPIC’s electronic medical record and MyChart patient portal from Houston Methodist.  Please contact the Houston Methodist MyChart customer service team at 832.667.5694 #1 for help or additional information.

COMMUNICATE ELECTRONICALLY - Send digital messages from the comfort of home

    Send new, or view previous, messages to your doctor’s team:

    • Request appointments
    • Ask a medical question
    • Refill a medication
    • Ask a customer service question

    Not currently available: actual scheduling of an appointment. Call your doctor for same day, work-in, or overflow appointment availability, whether for an in-person or video visit.

APPOINTMENTS

    • View details about upcoming appointments with your provider

    But … Requests to re-schedule or cancel upcoming appointment are made thru the reminder link received via text or email

    Use the “Messages” function to request potential dates / times for new appointments

    • Save time at your next appointment by completing eCheck-In details
    • Enter your information up to 48 hours in advance of your appointment time

REMINDERS

    • View notifications of procedures and/or immunizations recommended for you

TEST RESULTS

    View Your Results!  You’ll receive a notification (text/email) about new results, typically within 24 hours.  View:

    • Lab results – current & prior (for comparison purposes)
    • Radiologists’ interpretive reports
    • Interpretive reports of cardiac and other ancillary procedures

    Critical results will initiate a call from your provider within 24 hours – please call your provider to follow-up!

    There are abnormal results which are considered minor and not clinically significant.  These “non significant” results will be addressed within 1-2 weeks

REQUEST PRESCRIPTION REFILLS

    • Request prescription refills from your provider who wrote the prescription

    We also recommend you request refills from your pharmacy, who will electronically send the request to the prescribing doctor

AFTER VISIT SUMMARY

    • View your after “Visit Summary” and “Clinical Notes” written by your doctor, related to your visit(s)
    • View Medication Instructions & Changes, and confirm which pharmacy provides your medications
    • Review your documented Allergies, Health Issues, and Medications

Your privacy is paramount. With MyChart, hosted by Houston Methodist, you set the permissions for sharing medical information, so only your health care team and those you designate will be able to view your records.

SHARE YOUR INFORMATION / ASSIGN PROXIES

    In the “Sharing Hub” you identify who you want to have access to your MyChart information including

      • Family member, close friend, or caretaker
      • Proxy
      • Another organization like your school, insurance, or workplace
      • Other healthcare providers

UPDATE PERSONAL INFORMATION

    Update your:

    • Address & contact numbers
    • Medications
    • Allergies
    • Immunizations
    • Contacts
    • Proxy and other access

    Updates to insurance information must be phoned into MCH Business Office at 713.520.4700, M-F 9:00 am to 4:30 pm
