We understand Dr. Finnila’s March 1, 2026 transition to full concierge practice model may be unexpected and may raise questions about your ongoing care.  This page is an guide for his MCH patients who wish to choose a new primary care physician (PCP) with a traditional practice model.

 

We will do everything we possibly can to ensure the seamless transfer of your care to one of these MCH physicians listed below, while retaining your medical records, test results, and Dr. Finnila’s clinical notes. We are confident that these physicians will continue to provide the same high-quality care you have come to expect.

♥  Comprehensive Care:       We are committed to providing thorough medical work-ups which include on-site diagnostic testing and procedures, often offered same-day, to accurately diagnose and treat your medical conditions.  Our onsite services include: Radiology, Non Invasive Cardiology, Laboratory, Infusion, and other screening tests and procedures.

 

♥  Personalized Attention:   Our staff is dedicated to providing compassionate and attentive care, ensuring your needs are met. Our “MyChart” patient portal remains available to you to securely access test results, physician notes, on-line appointment check-in, and communicate with your care team.

 

♥  Accessibility                       Our physicians are readily available, even for urgent medical concerns, ensuring you have timely access to the care you need. Same day or next day appointments are available by a phone call and your results are accessible via the MyChart patient portal. MCH hospitalists collaborate with your primary care and subspecialty doctor(s) at MCH to manage your care from admission to discharge.

Demonstrating our commitment to provide ongoing care to you, the following MCH PCPs are opening their practice to you, regardless of whether their profile on MCH’s website reflects that their practice may be closed.

The “MCH Internists” section displays those physicians who will welcome current MCH patients into their traditional practices. →  To a new non-MCH PCP: Identify and contact a non-MCH physician who is accepting new patients.

Should you choose to obtain primary care outside of MCH

A copy of your medical records can be transferred to another physician only with your written permission and upon receipt of the cost for copying and mailing.

Before records can be released, please follow the instructions (below) which are required for your request to be completed.

      1. A completed and signed Medical Records ‘Release of Information’ form must be provided to HIMS.
      2. This completed and signed form may be submitted to MCH HIMS via:
        • Fax: (713) 526-0451
        • U.S. Postal Service:  Medical Clinic of Houston, L.L.P.  Attention: HIMS,  1701 Sunset Blvd, Houston, TX 77005
        • Email (non-secure):   roi@mchllp.com.  If choosing email, we encourage you to send your documents encrypted as an attachment. In advance of sending the document, be sure to alert medical records how to open the encrypted email document.
      3. Fees may apply, according to applicable Texas Consumer / Public Regulatory laws and regulations. Should fees apply, within 15 business days of receiving the completed and signed Medical Records ‘Release of Information’ Authorization form, an invoice will be mailed, via United States Post Office, to the responsible party.
      4. Payment may be called in or sent to MCH HIMS.  Methods can include cash, check, or credit card.
      5. Within one to two business days of receipt of payment, records will be sent via method (certified mail, fax, or pick up) identified on ‘Release of Information’ Authorization form.
