Signing up for MyChart is simple. Request an activation code from your doctor’s secretarial staff or check-in staff during your visit or go to the MyChart website and select “Sign Up Now.”
MCH leases EPIC’s electronic medical record and MyChart patient portal from Houston Methodist. Please contact the Houston Methodist MyChart customer service team at 832.667.5694 #1 for help or additional information.
This Mychart function is linked only to Houston Methodist Hospital’s bills.
Important Note on “MyChart” Functionality
Because MCH leases the MyChart patient portal from Houston Methodist Hospital, several of the displayed features are not available to Medical Clinic of Houston patients.
While MyChart is your portal for viewing test results and communicating with your care team, please be aware that some features shown are not active for MCH patients.
To perform the following actions, please contact MCH directly – by phone:
Schedule urgent “Same Day” appointments (Non-urgent appointments may be requested through MyChart “Message”)
Update your insurance information
Schedule mammograms and other ancillary services (Request your provider to schedule on your behalf)
After May 2023, your visits with an MCH physician, along with your clinical information
can easily and securely be viewed from your personal MyChart app,
anywhere and anytime.

Under ‘Visits” icon, view details about upcoming appointments with your provider
But … If you desire to reschedule or cancel upcoming appointment(s), your request must be made thru the “reminder link” received via your text or email. A follow-up phone call to your doctor’s secretary is necessary to finalize the rescheduling or cancellation.

You will receive an eCheck-in link (via text or email) 48 hours in advance of your scheduled appointment. Click on the embedded link to access your “MyChart” check-in details.

Health Summary
Health Summary is a centralized menu item which allows individuals to View, Remove, and/or Update multiple items.
It also includes “Quick Links.”
To access this menu item:
Log into your MyChart App, from the ‘Main Menu (found on the top left corer), scroll down to (or search for) “My Record” then click on “Health Summary“
NOTE: Do NOT use the “Request appointment” button. Instead call or send a “message” to request an appointment with your doctor’s office.
HEALTH SUMMARY options are displayed from this single menu and include your:
Condition Summaries (View each condition documented by your doctor)
Medications (to View and/or Update, click “Go to Medications” or use the Medications icon)
Test results (to View, click the “Go to Test Results” or use the “Results” icon)
Education(View recommended reading and directions for patients related to their conditions)
Current Health Issues (to View, Remove, or Add, click on “Go to Health Issues”)
Allergies (to View, Remove, or Add, click on “Go to Allergies”)
Immunizations (to View your list of immunization doses we have on file for you, click on “Immunizations”)
Recommended Actions (View or Update details of preventive care items recommended by your physician; Update by marking item as complete and indicating date completed) Call to schedule an appointment with your doctor’s office – do NOT use the “Request Appointment” button.
Share My Record– links to the “Sharing Hub” (View, Remove, or Add, specifying who you would like to share your health information with.)

REMINDERS
From the Mychart “Welcome” page
View notifications of procedures and/or immunizations recommended for you
TEST RESULTS
From the “Test Results’ icon, view Your Results! You’ll receive a notification (text/email) about new results, typically within 24 hours. Options include :
Lab results – current & prior (for comparison purposes)
Radiologists’ interpretive reports
Interpretive reports of cardiac and other ancillary procedures
Critical results will initiate a call from your provider within 24 hours – please call your provider to follow-up!
There are abnormal results which are considered minor and not clinically significant. These “non significant” results will be addressed within 1-2 weeks
REQUEST PRESCRIPTION REFILLS
Under “Medications” icon, request refills directly from your prescribing provider.
We also recommend requesting refill(s) though your dispensing pharmacy, who will electronically send the request to your prescribing provider
Visits and Medications
From your myChart “Welcome” page:
Under the “Visits” icon: view your “After Visit Summary” and “Clinical Notes” written by your doctor, related to your visit(s)
Under the “Medications”icon: view Medication Instructions & Changes, and confirm which pharmacy provides your medications
Your privacy is paramount. With MyChart, hosted by Houston Methodist, you set the permissions for sharing medical information, so only your health care team and those you designate will be able to view your records.
SHARE YOUR INFORMATION / ASSIGN PROXIES
In the “Sharing Hub” (found under “Explore More”) you can specify who you would liketo have access to your MyChart information including:
Family member, close friend, or caretaker
Proxy
Another organization like your school, insurance, or workplace
Other healthcare providers
UPDATE PERSONAL INFORMATION
Under “Menu,” then “My Record,” you may update the following information:
Address & contact numbers
Medications
Allergies
Immunizations
Contacts
Proxy and other access
NOTE: Insurance updates must be submitted – by phone – to the MCH Business Office at 713.520.4700, ext. 4700, Monday through Friday between 9 A.M. and 4:30 P.M.
